ABSTRACT

Abstract Intracapsular proximal femoral fracture is a frequent injury in elderly patients, often associated with low-energy trauma and reduced bone mass. In young patient, it is uncommon, usually caused by high-energy trauma and accompanied by damage to the adjacent soft tissues. However, reports of open intracapsular proximal femoral fracture due to indirect trauma are rare in the orthopedic literature. In the present article, we describe a case of this injury in a 35-year-old man involved in a car accident. The proximal femur was exposed at the gluteal region due to a mechanism similar to dislocation of the posterior hip. We describe the initial treatment and subsequent management until achieving a definitive solution using total hip arthroplasty and muscle transfer to reconstruct the abductor mechanism of the hip. At 10 months of follow-up, the patient presented good functional outcome, with gradual recovery of the abductive strength and a Harris Hip Score of 91 points. In addition, a radiographic study showed that the cemented total prosthesis was well-positioned. This therapeutic strategy (total hip arthroplasty with muscle transfer to reconstruct the abductor musculature) was successful to treat an intracapsular proximal femoral fracture with bone exposure.


Resumo A fratura intracapsular do fêmur proximal é uma lesão frequente no paciente idoso, e em geral está associada a trauma de baixa energia e redução da massa óssea. No jovem, esta lesão é pouco frequente, decorre de trauma de alta energia, e resulta em dano das partes moles adjacentes. Contudo, o relato de fratura intracapsular do fêmur proximal com exposição óssea por trauma indireto é raro na literatura ortopédica. Neste relato, esta lesão foi diagnosticada em um homem de 35 anos, vítima de acidente automobilístico. Mediante um mecanismo semelhante ao da luxação posterior do quadril, o segmento proximal do fêmur determinou exposição óssea através da região glútea. Foram descritos o tratamento inicial e os tratamentos subsequentes até a solução definitiva por artroplastia total do quadril associada a transposição muscular para reconstrução do mecanismo abdutor do quadril. Após 10 meses de seguimento, o paciente apresentava boa recuperação funcional, com retorno gradual da força abdutora, Harris Hip Score de 91 pontos, com estudo radiográfico revelando prótese total cimentada bem posicionada. A estratégia terapêutica utilizada neste paciente (artroplastia total do quadril com transferência muscular para a reconstrução da musculatura abdutora) foi uma solução eficiente para tratar a fratura intracapsular do fêmur proximal com exposição óssea.

ABSTRACT

Introdução: A depressão é uma doença complexa, em que seu surgimento e desenvolvimento são marcados pela influência de diversos fatores. Os estudantes do curso de medicina tendem a compreender um grupo vulnerável à doença, visto que os mesmos lidam diariamente com fatores estressores durante toda a graduação. Objetivo: Estudar a prevalência dos sintomas do transtorno depressivo maior nos discentes de medicina de uma instituição de ensino superior no estado de Alagoas, nordeste do Brasil. Metodologia: Estudo epidemiológico elaborado com a participação de 259 estudantes do curso de medicina de uma instituição de ensino superior do nordeste do país, no período de agosto de 2019 e julho de 2020. O instrumento de coleta dos dados foi um questionário sociodemográfico e o Inventário de Beck (IDB). Resultados: Foi encontrada, no estudo dos sintomas depressivos nessa população, a prevalência de 51,80%. Sobre a realização de tratamento psicológico e psiquiátrico, grande parte respondeu jamais ter procurado ajuda profissional, apesar de 29,3% fazer uso de algum psicofármaco. Conclusão: As informações coletadas poderão ser utilizadas para contribuição dos dados epidemiológicos do país, de modo a propiciar melhorias na formação dos estudantes da graduação, uma vez que o reconhecimento do problema e suas variáveis prevalentes poderão determinar novas abordagens, bem como a conscientização acerca dessa patologia e de seu cuidado adequado. Introduction: Depression is a complex disease, and its onset and development are influenced by several factors. Medical students represent a group that is vulnerable to the disease, as they deal with daily stressors throughout their undergraduate studies. Objective: To study the prevalence of symptoms of major depressive disorder among medical students at a higher education institution in the state of Alagoas, northeastern Brazil. Methodology:Epidemiological study carried out with the participation of 259 medical students from a higher education institution in the northeast region of the country, from August 2019 to July 2020. The data collection instruments were a socio-demographic questionnaire and the Beck Depression Inventory (BDI). Results: In this study, the prevalence of depressive symptoms was 51.80%. Regarding psychological and psychiatric treatment, most students answered that they had never sought professional help, although 29.3% were using psychotropic drugs. Conclusion: The information collected can contribute to the country's epidemiological data, enabling improvements in the training of undergraduate students, as the identification of the problem and its associated variables may help in the development of new approaches and raise awareness about this pathology and its proper care. ABSTRACT

Introdução: em estudos de gênero que têm como foco a Educação Física (EF) escolar, identifica-se a predominância de um discurso crítico dirigido aos professores de EF atuantes na educação básica. Tal discurso se destaca pela denúncia sobre a falta de conhecimento e de competência para interferir nas questões de gênero evidentes no cotidiano escolar. Objetivo: este estudo de caso buscou responder à questão: como o curso de licenciatura em EF de uma universidade privada está formando os futuros professores para lidarem com as questões de gênero na educação básica? Método: os dados reunidos pela pesquisa documental e as entrevistas com dez professores universitários e oito estudantes concluíntes, foram analisados a partir dos referenciais teóricos e metodológicos da Teoria de Gênero, Teoria Representações Sociais e Análise do Conteúdo. Conclusão: conclui-se que a temática de gênero não está devidamente contemplada pelo currículo da licenciatura em EF, ficando à margem, sem planejamento no âmbito das disciplinas da graduação.


Introduction: in gender studies with focus in Physical Education one can identify a critical discourse targeting teachers working in the basic school. This discourse stressed the lack of informationand skills to discuss gender issues, a common aspect of the school realil y. Objective: this case study aims to answer the following question: how well is the undergraduate course in Physical Education of a private university priming future teachers to cope with gender issues in basic education? The data collected from documental research and from interviews with ten university professors and eight students were analyzed by the theoretical frameworks of Gender Theory, Theory of Social Representations and the Content Analysis. The results showed that the gender issue has not been contemplated by the Physical Education undergraduate curriculum and is found to be without planning or support in the undergraduate courses.

Abstract Oral complications of RA may include temporomandibular joint disorders, mucosa alterations and symptoms of dry mouth. The aim of this study was to evaluate the salivary gland function of subjects with rheumatoid arthritis (RA) comparing it to healthy controls. Subjects with other systemic conditions known to affect salivary functions were excluded. A questionnaire was applied for the evaluation of xerostomia. Resting and chewing-stimulated salivary flow rates (SFR) were obtained under standard conditions. There were 145 subjects included of the study (104 RA and 38 controls). About 66.7% of the RA subjects and 2.4% in control group presented xerostomia. The median resting SFR were 0.24 ml/min for RA subjects and 0.40 mL/min for controls (p = 0.04). The median stimulated SFR were 1.31 mL/min for RA subjects and 1.52 ml/min for controls (p = 0.33). No significant differences were found between resting and stimulated SFR of RA subjects not using xerogenic medications and controls. There was significantly higher number of subjects presenting hyposalivation in the RA group than among controls, even when subjects using xerogenic medications were eliminated from the analysis. In conclusion, hyposalivation and xerostomia were more frequent among RA subjects not using xerogenic medication than among controls, although there were no significant differences in the median SFR between groups.

ABSTRACT

OBJETIVO: Avaliar a segurança do uso de fixação iliosacral com parafusos em pacientes com lesão do anel pélvico. MÉTODOS: Trata-se de um trabalho restrospectivo analisando por tomografia computadorizada a topografia de 60 parafusos inseridos em 46 pacientes portadores de lesões do anel pélvico e operados no INTO-RJ, no período de 2006 e 2010. RESULTADOS: Em 21,7 por cento dos casos o parafuso encontrava-se fora do corredor de segurança descrito em literatura. Destes, 77 por cento estavam associados a uma redução insatisfatória do anel pélvico. Não houve casos de lesão neurovascular em nossa série. CONCLUSÃO: O uso de parafusos ílio-sacrais é uma técnica segura, pois mesmo em casos onde o parafuso não respeitou uma topografia ideal, não houve complicações significativas associadas a este método. A má redução do anel pélvico é um fator muito importante associado ao mau posicionamento dos parafusos.


OBJECTIVE: To evaluate the safety of iliosacral screw fixation in pelvic injuries. METHODS: A retrospective study based on computer tomography evaluation of screw topography and its relationship with the so called pelvic safe corridor. RESULTS: We evaluated the topography of 60 screws inserted in 46 patients sustaining pelvic injuries which were operated in the INTO from 2006 to 2010. In 21.7 percent of cases the screw was located outside the safe corridor. 77 percent of these misplaced screws were associated with an unsatisfactory reduction of the pelvic injury. CONCLUSIONS: In our study, a significant rate of misplaced screws in relationship with the safe corridor has been pointed out and in most of these cases an insufficient reduction of the pelvis was also identified. We conclude that iliosacral fixation is a safe adjuvant method for pelvic fixation since in our series even with a relative high incidence of misplacements, no neurovascular injuries have been recorded in association with this technique.

OBJETIVOS: O propósito do presente estudo é revisar aspectos epidemiológicos das fraturas desviadas do acetábulo tratadas cirurgicamente no Instituto Nacional de Ortopedia e Traumatologia (INTO). MÉTODOS: Analisamos, retrospectivamente, 126 fraturas de acetábulo tratadas cirurgicamente no INTO, no período de março de 2006 a novembro de 2008. Levamos em consideração: idade; sexo; mecanismo de trauma; classificação da lesão; tempo decorrido entre o trauma e o tratamento cirúrgico; lado acometido; lesões ósseas associadas. RESULTADOS: 76,8 por cento são do sexo masculino, a média da idade foi de 39,6 anos; quanto ao mecanismo de trauma, 59 por cento foram por acidente automobilístico; o tempo decorrido entre a lesão e o tratamento cirúrgico foi em média 16,4 dias; 55 por cento casos foram do lado direito; 30 por cento pacientes apresentavam fraturas associadas. CONCLUSÃO: A maior parte dos pacientes é do sexo masculino, na faixa etária economicamente ativa, e vítima de acidente de trânsito, sendo as fraturas da reborda e/ou coluna posterior as mais frequentes. Lesões associadas são comuns e a maior parte das fraturas operadas em nosso serviço é envelhecida.


OBJECTIVES: The purpose of this study is to review epidemiological aspects of displacement fractures of the acetabulum treated surgically at the Instituto Nacional de Traumatologia e Ortopoedia (INTO). METHODS: We retrospectively analyzed 126 acetabulum fractures treated surgically at the INTO in the period from March 2006 to November 2008. The following elements were taken in account: age, sex, mechanism of injury, classification, time elapsed between trauma and surgery, affected side, associated bone injuries. RESULTS: 76.8 percent were male, the mean age was 39.6 years. 59 percent of the traumas were due to traffic accidents; time between injury and surgery was on average 16.4 days; 55 percent of cases were on the right side; 30 percent of patients had associated fractures. CONCLUSION: Most patients are male, in an economically active age group, and are victims of traffic accidents. Fractures of edge and/or posterior column are the most frequent. Associated injuries are common and most of the fractures operated in our service are delayed.

ABSTRACT

Endoglucanase and exocellobiohydrolase produced b Lentinula edodes (Berk.) Pegl. strain thatt was cultivated in carboxymetilcellulose (CMC) or microcrystalline cellulose (Avicel) liquid media. The concentration and type of cellulose influenced the enzyme activity and production. Extra-cellular cellobiase activity was not detected in CMC or Avicel media. This enzyme was detected in mycelial extracts only. With 1.7 percent Avicel liquid medium, the strain did not produce exocellobiohydrolase, but 74 æmol RBBR/mg protein/min was detected with 0.5 percent Avicel. The substitution of Avicel by 0.5 percent CMC reduced this activity. Endoglucanase also had maximum activity in 0.5 percent Avicel medium (approximately 820 UI/mg protein) after 96 h incubation. In supernatants from 0.5 percent CMC, the maximum activity attained was 200 UI/mg protein only

ABSTRACT

Various species of mushrooms belonging to genus Pleurotus (Fr.) Kumm. are known by its nutritional value and medicinal properties, particularly anti-tumor activity and immunomodulation, related to structural polysaccharides. In order to study the composition and the effects of aqueoussoluble constituents from Pleurotus ostreato-roseus Sing. on mice serum proteins, the P. ostreato-roseus strain POR-020 was cultured on powered grass-sugarcane bagasse-sawdust-wheat bran-substrate, and fruiting bodies were produced, milled, extracted with ethanol and hot water, and fractional precipitated with ethanol producing two fractions, FI-1 and FI-2. These fractions showed, respectively, 47.87 and 26.45 percent (d. m.) carbohydrate, and 2.07 and 1.97 percent (d.m.) protein composition. The fractions were administered i.p. in AJ male mice in a singledose of 257 mg/kg (FI-1) and 31.5 mg/kg (FI-2). After 15 days administration, electrophoretic analysis of serum proteins detected that FI-1 increased 50, 25, 0.35, and 28.6 percent in b-, a1-, a2- and g-globulin regions, and FI-2 increased 4.4 and 21 percent in band a1-globulin, respectively.

ABSTRACT

A modified method for direct determination of cellulolytic activity using Avicel colored with Remazol Brilliant Blue R (RBBR) in Agar test tubes in discussed. Refinaments were introduced in a simple method for qunatitation of cellulase activity, based on the release of dye from Avicel-RBBR medium by enzimatic hydrolysis. Modification in Avicel-dye preparation were enhanced and a spectrophotometer improved the precision of the collected data, since absorbance measurements could be done at the maxmum wavelenght for RBBR (595 nm).

ABSTRACT

Foi avaliada radiologicamente a incorporaçao óssea femoral em 39 prótese bipolares nao cimentadas, realizadas em 31 pacientes portadores de necrose avascular (NAV) da cabeça femoral, num período de cinco anos e três meses (seguimento médio de três anos e três meses), utilizando critérios de incorporaçao óssea idealizados por Charles Engh para avaliaçao radiológica. Foi observada incorporaçao do componente femoral em 88 por cento das próteses, demonstrando ser opçao de tratamento para uma doença que teoricamente apresenta fisiopatologia que interfere no processo de fixaçao e estabilidade das próteses nao cimentadas.
